The theme of DFTB17 was ‘Making a Difference‘, and it was important for us to close with a story of hope, a story of how we can impact the lives of others. Josh Francis is a paediatric infectious diseases specialist at the Royal Darwin Hospital. He has been working alongside the people of Timor-Leste since 2013 to help combat rheumatic heart disease. This is a talk about children, hope, and the triumph of the human spirit.

In the world of medicine, our capabilities to transform lives extend far beyond the borders of our own communities. As medical professionals, we have the privilege and responsibility to contribute to positive change on a global scale. One such region that demands our attention and expertise is East Timor, a small Southeast Asian country with significant healthcare challenges.
Setting the Stage: The Urgency of Change
In a thought-provoking talk, Dr Josh Francis addresses an important question: Does the world need to change? While many of us enjoy the comforts of modern healthcare and technology, there are parts of the world, like East Timor, where access to basic medical care remains a struggle. This is particularly true for the vulnerable pediatric population.
The Pediatric Landscape in East Timor
For those who are geographically challenged, Timor-Leste is located in maritime South-East Asia. It has a population of around 1.3 million and an infant mortality rate of 35.1 deaths per 1000 live births.
East Timor, also known as Timor-Leste, is a country located in the southeastern part of Asia. Despite its natural beauty and rich cultural heritage, the nation grapples with critical healthcare challenges that disproportionately affect its children. Staggering statistics reveal that Timor-Leste has the highest stunting rate in children under five globally, with up to 60% experiencing moderate to severe stunting. Malnutrition is a pervasive issue that threatens the growth and development of young lives.
The Fight Against Preventable Diseases
While the challenges in East Timor are numerous, one poignant story underscores the urgency of the situation. Meet Pharrell, an eight-year-old boy whose life was profoundly impacted by the lack of access to quality healthcare. Pharrell’s journey began with chronic headaches, vomiting, and weight loss, culminating in a harrowing diagnosis of cryptococcal meningitis. However, due to limited healthcare resources, it took four months to diagnose his condition accurately. Even after diagnosis, the barriers to proper investigation and management persisted, highlighting the dire need for change.
A Call to Action: Changing the Landscape of Pediatric Care
Dr Francis] acknowledges that change is possible, emphasizing the transformative power of investing in people. In the context of East Timor, this investment takes various forms, each contributing to a brighter future for the country’s children.

In the words of Dr Francis, “Unless we are willing to be like that Good Samaritan, to say, ‘Alright, this one’s here in front of me, I’m going to invest in them,’… I don’t think we have any hope of changing the world.”
